Services may be covered in full or in part by your health insurance or employee benefit plan. Please check your coverage carefully by asking the following questions:

What is my deductible and has it been met?

Payment
We accept cash, check, and all major credit cards as forms of payment.
Cancellation Policy
If you do not show up for your scheduled appointment and have not notified us at least 24 hours in advance, you may be subject to cancellation fees.

To schedule an appointment or to obtain additional information about any of our medical services, please give us a call in advance. For new patients please try to call us ahead of time so that we have time to process information that we will request from you, so upon arrival any confirmation procedures can be completed accurately.
