What should I bring to my first appointment?
Please make sure that patients arrive to the clinic at least 15 minutes before your scheduled appointment time so that you can provide us the necessary health and insurance information to put you into our system. To save time, patients can also print the new patient forms located on our appointments page. It is recommended to bring them filled out to your appointment.
Do I need a referral to make an appointment?
For most commercial insurance, patients do not need a referral to make an appointment with a rheumatologist but it is very helpful to have any recent imaging test or labs results available at the time of the appointment. However, if a patient does have any of their test results, it makes it easier to give a more detailed consultation based on their condition. For a patient's visit to be covered through insurance please contact your insurance provider to understand the terms of their policy. For Medicaid patients a referral from your primary care provider is mandatory.
How are the relationships different here, compared to other hospitals or clinics?
We believe that our staff members are leaders in making our patients as happy as they can so they can get back to what they love doing the most. We take the time to listen to patients’ concerns and address any questions that they may have. We are a clinic that values all of our patients and appreciate the long-lasting relationships we create with them.
How will I pay for my care?
If you have insurance, please bring in your card and we will either collect your co-pay or bill you through your insurance plan. If you are not insured, we require payment at the time of service. We strive to commit all aspects of our practice, including our professional fees, which reflect our ethical and rational approach to your care.
What is your cancellation policy?
We require a 24 hour notice to cancel all appointments. Please be sure to contact us if patients will not be able to make their scheduled appointment to allow other patients to be accommodated into the clinic schedule.
Will I be able to get copies of my test results?
Yes, upon request we are able to provide patients copies of their test results.
Do you allow urgent or same day appointments?
We do set aside time for urgent visits. However, if patients are undergoing an emergency, please dial 9-1-1 immediately.
